The Realities of Working as an Expedition Nurse, by Ship Nurse, Garry de Guzman

When people hear that I work as an expedition nurse, they often imagine breathtaking glaciers, penguins, polar bears and endless adventures in some of the most remote places on Earth.

These experiences are certainly part of the job, but they represent only a small part of what life is really like.

Being an expedition nurse is first and foremost about caring for people. Whether we are sailing through Antarctica or exploring the Arctic, the ship’s medical team is responsible for the health and wellbeing of everyone on board. Every day brings new challenges and no two days are ever the same.

Unlike working in a hospital, there is no team of specialists just down the corridor. In remote regions, the Medical Centre may be the only healthcare facility available for hundreds of miles. An expedition nurse must therefore be prepared to assess, treat and stabilise a wide range of medical conditions with limited resources. Sound clinical judgement, adaptability and the ability to remain calm under pressure are essential.

The role also extends far beyond responding to emergencies.

Much of our work involves preventive medicine, health promotion, infection control, public health surveillance, medication management and medical documentation, as well as ensuring compliance with maritime health regulations. Crew members and guests rely on the medical team when they are ill or injured, but they also depend on us for information and support throughout the voyage.

Life on board also requires flexibility. Expedition nurses work long hours and remain on call for emergencies at any time of the day or night. Medical situations do not follow a schedule. Within minutes, we can go from treating a minor cut to managing a life-threatening emergency.

Despite the stresses and demands of the role, maintaining professionalism and compassion is essential.

Another reality people rarely see is the emotional impact of the job. Working in isolated environments often means being away from family and loved ones for months at a time. Holidays, birthdays, anniversaries and other important milestones may be spent apart.

Staying connected with loved ones while remaining focused on the people under our care requires resilience and emotional strength.

Yet despite these challenges, the rewards are extraordinary. Few healthcare professionals have the opportunity to provide care while witnessing towering icebergs, spectacular wildlife and some of the planet’s most pristine landscapes. Watching whales surface beside the ship, seeing penguins in Antarctica or spotting polar bears in the Arctic are experiences that never lose their sense of wonder.

The relationships formed on board can be equally rewarding.

Crew members come from many different countries and cultures, creating a truly international community. Living and working together fosters teamwork, mutual respect and friendships that often last well beyond a single contract.

Working as an expedition nurse also provides continuous opportunities for professional growth. The unique environment strengthens clinical decision-making, emergency preparedness, leadership, communication and resource management. Every voyage presents new challenges and lessons that contribute to both professional and personal development.

Ultimately, being an expedition nurse is about far more than having a job with an extraordinary view. It is a profession built on adaptability and compassion. Behind every stunning photograph of glaciers or wildlife is a healthcare professional ready to respond when someone needs help.

For those considering this career, it’s important to understand that while the work can be demanding and unpredictable, it can also be extraordinarily fulfilling for nurses with a spirit of adventure and a commitment to caring for others in challenging environments. It is a truly unforgettable career.
