By clicking on the I AGREE & GIVE CONSENT as representative of CMI, CMI agrees with the statements below and consents to the review of their passenger’s medical records as indicated in the Passenger Medical Records Privacy Policy:

I have read the Passenger Medical Records Privacy Policy and consent to Vikand’s receipt, use, sharing and storage of the medical records for the purpose of review prior to embarkation. By submitting passenger medical records, CMI agrees that data is administered by Vikand for this purpose for a period of 7 years.

This consent can be withdrawn at anytime by submitted a written request to info@vikand.com.
